01Choose a pathway with a clear finish
Start with a familiar type of follow-up, a defined review point and an agreed episode owner. A broad invitation to send updates without a review routine creates additional work rather than a reliable service.
02Set up the routine
- Agree who prepares and approves the plan before the owner leaves.
- Reserve review time and define cover for absences.
- Tell owners the response hours and keep the usual urgent contact route visible.
- Decide what is recorded in the PIMS and when the care episode closes.
03What to assess during the free month
Measure invitation completion, missing check-ins and staff review time for comparable cases. Include all involved roles in the review so the process works for reception as well as the clinical team. All currently available features are included.
